Short description of project idea : 
The design proposal is dedicated to diabetic patients (type 2 diabetes) and aims to offer a new user experience through the implementation of an APP, both for pathology monitoring and for thefull-digital communication between patient and specialist, proposing to intercepting needs/demands and having a proactive approach to the day-to-day management of diabetes.

Main areas of expertise: 
ProMIS main tasks can be summarized as follow: a)offering opportunities for information, dissemination and training; b) fostering the capacity building ofpublic administrations at any level of governance of the national social-healthcare system; c) respondingthe needs jointly expressed by the regions and local social-healthcare authorities; d) stimulating theparticipation of Italian clusters to national and European consortia and transnational initiatives; e)supporting the regions and other public administrations in transferring knowledge and competencesacross the network and beyond at EU level. ProMIS ensures effective and continuous connection withthe EU institutions securing exchange of information, scaling up of innovative practices and participationto specific European networks and multipliers initiatives. More specifically, ProMIS has facilitated the preparatory activities and follow up to support Regions and local healthcare stakeholders to participateto the European Innovation Partnership for Active and Healthy Ageing (EIP on AHA), including the application to become a Reference Site. In fact, since its beginning ProMIS has promoted opportunities of confrontation among the Italian regions about themes concerned with active and healthy ageing,also stimulated by the cooperation developed in the international context and coherent to it. ProMIS has systematically offered opportunities of information and discussion, organizing more than 10 national & international workshops and conferences, satisfying the needs jointly expressed by the regions. Moreover, ProMIS keeps a continuous connection with the communitarian institutions, granting the participation of a representative of the European Commission and/or the European networks tothe specific events. Finally, ProMIS has developed preparatory activities to help Italian Regions apply to the call for theReference Sites and for the commitments, explaining the details of the calls, facilitating theaccess to the useful information in order to prepare the proposal, thus making the Italian Regionscompete at their best with the other European Regions. To conclude, Italian Regions agreed to draft a document focusing on their organisation methodology in order to provide a complete document to EIP-AHA which explores Italian Reference Sites organizationa lsystems describing their management, methods and tools. It relies on validation elements of the RSs’model to be shared at European level.
Main objectives of the project and how will they be achieved: 
Description: 
The project has a dual objective: • From the clinical point of view, the goal is to make the blood glucose monitoring more safe, accurate and automatic, with a constant feedback system for the over threshold values set by the specialist • From the assistance point of view, the purpose is to experiment a new patient-specialist communication channels that can lead to a new organizational models for delivering the monitoring services for chronic disease such as diabetes. At the technological level, the idea involves the development of an APP, integrated with the regional EHR, the medical records and the monitoring devices (for example the glucometer) with two functions: • For the physician, the opportunity to establish a safe and direct communication channel with the patient (remote activity), which also includes, from a clinical and “administrative” point of view, the tracking of health services (recording of outcome and monitoring):this system would allow the tracking and the identification of these activities as telemedicine performances (also for example for activities out of the normal working hours). Furthermore, it would be possible to share the instrument / interface with other professionals involved in the clinical pathway of the patient. • For the patient, the possibility to have a unique tool for the automatic download and loadof personal data, avoiding manual insertion (and possible consequential errors). The service could also include an alert system linked to the measured clinical parameters and the lifestyle of the patient.
