Short description of project idea :
NextHealth intends to use personalised medicine strategies to create predictive models of diseases and innovative and sustainable organisational processes to target disease prevention and treatment in a more efficient and effective way. In order to achieve this, NextHealth will setup a series of longitudinal studies implementing large pilots in at least three European Countries to validate the effectiveness and cost-benefit of the proposed solutions.
Main areas of expertise:
ProMIS main tasks can be summarized as follow: a)offering opportunities for information, dissemination and training; b) fostering the capacity building of public administrations at any level of governance of the national social-healthcare system; c) responding the needs jointly expressed by the regions and local social-healthcare authorities; d) stimulating the participation of Italian clusters to national and European consortia and transnational initiatives; e)supporting the regions and other public administrations in transferring knowledge and competencesacross the network and beyond at EU level. ProMIS ensures effective and continuous connection with the EU institutions securing exchange of information, scaling up of innovative practices and participationto specific European networks and multipliers initiatives. More specifically, ProMIS has facilitated the preparatory activities and follow up to support Regions and local healthcare stakeholders to participateto the European Innovation Partnership for Active and Healthy Ageing (EIP on AHA), including the application to become a Reference Site. In fact, since its beginning ProMIS has promoted opportunities of confrontation among the Italian regions about themes concerned with active and healthy ageing,also stimulated by the cooperation developed in the international context and coherent to it. ProMIS has systematically offered opportunities of information and discussion, organizing more than 10 national & international workshops and conferences, satisfying the needs jointly expressed by the regions. Moreover, ProMIS keeps a continuous connection with the communitarian institutions, granting the participation of a representative of the European Commission and/or the European networks tothe specific events. Finally, ProMIS has developed preparatory activities to help Italian Regions apply to the call for theReference Sites and for the commitments, explaining the details of the calls, facilitating theaccess to the useful information in order to prepare the proposal, thus making the Italian Regionscompete at their best with the other European Regions. To conclude, Italian Regions agreed to draft a document focusing on their organisation methodology in order to provide a complete document to EIP-AHA which explores Italian Reference Sites organizationalsystems describing their management, methods and tools. It relies on validation elements of the RSs’model to be shared at European level.

ACTIVITIES: - Current context conditions analysis in partners’ countries - Feasibility study from a legislative, programmatic and economic point of view - Identification of predictive models to determine clinical and economic key parameters - Identification of 4/5 pilot projects in partners’ countries which follow EBM therapeutic care diagnostic pathways based on predominant pathologies - Events organization involving local authorities in charge of health care, healthcare providers, healthcare system information management and utility structures - Compare, assess and validate outcomes and results (comparison with starting conditions and / or expected results) ) according to the models identified - Drafting of guidelines and recommendations OUTPUT: - Report on context analysis and feasibility with predictive models and pilot sites identifications - Report on individual pilot actions - Evaluation of overall outcomes/results - Recommendations/guidelines
